  • [32-09-5] A minimum of one bathroom facility is not available for public use. Public restroom sign covered with "out of order" sign. Manager stated the restroom facilities are not broken. Inspector had the manager remove the "out of order" sign during this inspection.
  • [36-37-5] Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Absorbent and porous acoustic ceiling tiles in-use throughout kitchen.
  • [29-34-4] Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Backflow prevention device missing at hose connection on side of building, near dumpsters.
  • [31B-02-4] No paper towels or mechanical hand drying device provided at handwash sink. No towels available at handwash sinks at front counter or in employee restroom. Manager replenished towels at both sinks during this inspection.
  • [33-11-4] Missing drain plug at dumpster. Drain plug missing from dumpster.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Single-use cup without handle stored down inside sugar inside bin in kitchen.
  • [08B-38-4] Food stored on floor. Plastic container of sugar syrup stored directly on floor in back of kitchen.
  • [41-07-4] Container of medicine improperly stored. Four prescription pill bottles stored on prep table beside and above food. Manager removed all medicine from the area during this inspection.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1. Exterior of ice machine soiled with lime-like substance. 2. Gasket on reach-in freezer door in back of kitchen soiled with mold-like substance.
