[25-05-4] Single-service articles improperly stored. Observed case of to go utensils stored on floor at front counter. Operator removed from floor during inspection.

[03B-01-6] Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed at cook line warmer top drawer: cooked rice (89F - Hot Holding). Bottom drawer: cooked rice (124F - Hot Holding); chicken breast (117F - Hot Holding). As per operator, items have been there less than 4 hours, operator turned up heat on machine.
