  • [14-31-5] Nonfood-grade bags used in direct contact with food. Nonfood-grade thank you shopping bags in direct contact with raw fish in walk in cooler and cut vegetables at back prep table.
  • [08A-05-6] Raw animal food stored over/not properly separated from ready-to-eat food. Raw chicken stored above ready to eat carrots cook line flip top cooler. Raw shrimp stored above cooked rice in walk in cooler. Operator reorganized.
  • [03F-02-5] Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. No time mark on sushi rice held at front sushi prep counter. Per operator, rice was prepared less than 1 hour ago. Operator applied appropriate time mark.
  • [41-10-4] Toxic substance/chemical improperly stored. Bottles of quaternary sanitizer tablets stored above in use food prep table by triple kitchen. Operator reorganized.
