  • [24-08-4] Equipment and utensils not properly air-dried - wet nesting. Metal pans of rack next to walk-in cooler stacked before properly air drying.
  • [36-73-4] Floor, wall and/or ceiling soiled/has accumulation of debris. Floor under soda machine in server station soiled with debris and black residue. Also, ceiling vent in back storage room soiled with dust. Also, floor in walk-in freezer soiled with food debris.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Reach in cooler fan cover soiled with dust build up. Also, under flip top lid of reach in cooler on cook line soiled with food debris.
  • [14-20-4] Ripped/worn tin foil used as shelf cover. On seasoning rack near cook line, shelves lined with ripped tin foil. Chef stated it will be changed today.
  • [35A-02-7] Live, small flying insects found. Observed one (1) live flying insect near soda machine in server station. Also, observed one (1) live flying insect near pizza oven.
  • [08A-02-6] Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In walk-in freezer, sealed bags of raw salmon stored on speed rack over three containers of heavy whipping cream. Chef moved whipping cream above raw fish.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Can opener attached to prep table on cook line soiled with build up. Chef stated it will be cleaned today.
