  • [25-17-4] Single-service articles stored on/in a soiled surface. Single-service wrapped straws and spoons stored in container with food debris under the dessert make line counter.
  • [29-18-4] Drain cover(s) missing. Drain cover missing in mop sink.
  • [36-73-4] Floor/wall/ceiling/vent soiled/has accumulation of debris. Vents at handwash sink by oil containers have dust. Floor under dessert area and under the fryers with food debris. Wall around dishmachine and floor under dishmachine is soiled.
  • [14-01-5] Bowl or other container with no handle used to dispense food. Take out cup used as a scoop in box of candies in the walk-in freezer.
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Filters on hood system were removed for cleaning, observed interior of hood system with grease build up. Shelf under flat grill with food debris/grease. Shelf under cold bar by drive thru has grease build up on the shelf and around the cooling unit.
  • [03A-02-5] Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Packaged noodles in the walk-in cooler at 47F. Per employee, noodles moved from the front line to the walk in cooler, noodles left out on the counter for a few minutes. Noodles moved to walk-in freezer to quickly chill and temperature retaken at 43F.
  • [11-26-1] No proof provided that some food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. No reporting agreement for Gregory who began work approximately one week ago. Inspector emailed Food Employee Reporting Agreement and operator had employee sign form during inspection.
  • [22-02-4] Food-contact surface soiled with food debris, mold-like substance or slime. Interior of far left soda nozzle with slime-like build up. Employee cleaned interior nozzle during inspection. Also, interior and exterior of ice chute on soda machine with black mold-like substance. Employee cleaned ice chute during inspection. Also, at top of spindles and on cup holder device on milkshake machine has dried food debris.
  • [50-17-3] Operating with an expired Division of Hotels and Restaurants license. License expired June 1, 2026.
  • [53B-14-5] Required employee training expired for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Food handler training for Andrew G expired on 1/20/2026.
