- [35A-03-4] Dead roaches/egg casings on premises. Observed one (1) egg casing on floor under handwash sink in kitchen. Employee cleaned up casing and discarded.
- [36-73-4] Floor soiled/has accumulation of debris. Floor in kitchen/under three compartment sink soiled with food debris.
- [22-08-4] Interior of oven/microwave has accumulation of black substance/grease/food debris. Interior top of microwave soiled with debris.
- [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Interior of multiple cabinets in lobby and kitchen soiled with food debris. Also, counter tops under both ovens in kitchen areas soiled with food debris.

Also, observed one (1) live roach on floor near doorway of kitchen. Employee killed roach and discarded.

- [16-43-4] Test kit for measuring the concentration of the sanitizing solution not used in accordance with the manufacturers instructions or employee unaware of proper use. Quaternary test strips at establishment expired 06/08.
