  • [13-07-4] Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed multiple employees engaging in food preparation wearing watches.
  • [08B-38-4] Food stored on floor. Observed multiple buckets of soy sauce on floor of kitchen. Observed cases of oil stored on floor of kitchen.
  • [10-20-4] In-use tongs stored on equipment door handle between uses. Observed in use tongs hanging on handle of kitchen oven. Employee removed and stored properly .
  • [23-03-4] Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed walk in cooler door gasket soiled with mold like substance.
  • [14-86-1] Non-food grade paper/paper towel used as liner for food container. Observed multiple food items raw fish, cut onions, cut meats lined with non food grade paper towels throughout kitchen and sushi bar.
  • [03F-02-5] Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined. See stop sale. Observed Sushi rice held on TIME As A Public Health Control without time mark. Employee placed time mark on sushi rice.
  • [41-10-4] Toxic substance/chemical improperly stored. Observed bottle of car wash cleaner stored on top of soda box located in back room. Employee removed and stored properly .
  • [31A-09-4] Handwash sink not accessible for employee use at all times. Observed employee handwashing sink blocked with bucket located next to ice machine. Manager removed and stored properly.
