Salmon determined to be out of temperature for approximately 2 hours. Broccoli cheddar casserole in walk-in cooler close to door at 44f-45f, determined to be out of temperature for approximately 3 hours. Operators applied ice baths and ice bags to rapidly chill items during inspection, bringing salmon down to 40f and broccoli cheddar casserole to 41f within 15 minutes.
  • Equipment in poor repair. Air conditioning in kitchen not functioning. Portable air conditioners are in use while awaiting repairs.
  • Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Interior of ice bin has mold-like substance in corners, along sliding door grooves, and in interior chute.
