- Equipment in poor repair. Ice bin in back in disrepair. Upon inspection metal sheet not flush with machine and allowing large hole into top of storage area of machine. Metal sheet is also bent in corner, and not allowing a complete closure of the top of the unit when pushed back in place.
- Plumbing system in disrepair. Hand washing sink pedals not properly functioning in kitchen. Sink pedals by glass front reach-in cooler not fastened to ground completely, not allowing sink to turn on at all times. Sink pedal missing on other side of wall from drive thru area, but still functional.

- Warewashing sanitizing solution exceeding the maximum concentration allowed. Upon inspection, 3 compartment sink quaternary sanitizer reading 500ppm. Per manufacturer instructions, sanitizer is to be 150-400ppm. Operator filled sanitizer sink with regular water and brought sanitizer down to 400ppm and called for recalibration of dispenser.
- Bowl or other container with no handle used to dispense food. Single use plastic cups and bowls used for scoops for seasoning, coleslaw, and dessert toppings. Operators removed cups and bowls during inspection.
- Equipment in poor repair. Upon inspection, dish machine digital display is damaged and unable to display information. Per operator, display has been broken for a week. Operator requested repairs during inspection.
