Observed flip-top cooler: diced tomatoes (60F - Cold Holding); cut lettuce (66F - Cold Holding); shredded cheese mixes (59F - Cold Holding) inside for approximately three hours per person in charge. Person in charge placed ice on product.

- Presence of insects, rodents, or other pests. Observed approximately 12 live moth-like insects inside two rubber containers filled with dried chili peppers. Observed approximately 4 live thin six legged crawling insects inside two rubber containers filled with dried chili peppers. Observed 2 live thin six legged crawling insects on floor of plastic storage shed, being used as dry storage, behind restaurant.
- Stop Sale issued due to adulteration of food product. Observed approximately 12 live moth-like insects inside two rubber containers filled with dried chili peppers. Observed approximately 4 live thin six legged crawling insects inside two rubber containers filled with dried chili peppers. Observed rice in black plastic grocery bag inside steam well. Operator cannot confirm black plastic bag is food safe.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cooked pasta in pasta station 57F at 11:47am. Per operator, item in cooling unit for less than two hours. Item out on ice baths. At 12:20pm, pasta down to 39F.
- Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Chlorine dishmachine not testing a minimum of 50ppm after multiple runs.
- Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Meatballs in hot holding well at 115Fat 11:46am. Holding hot for approximately 75 minutes. Operator reheated them, final temperature 177F at 11:51am.
