Operator stated cheese sauce was heated on cook top and moved to steam table 30 minutes prior to inspection. Operator began to reheat cheese sauce.
- Hot water sanitizing dishmachine final rinse not reaching proper temperature at manifold. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired. Dishwasher (Temperature 147)plate level ; Dishwasher (Temperature 172)manifold. Operator began to set up three compartment sink.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In the reach in cooler where hot food is prepared, Observed cooked chicken (47F - Cold Holding); cooked rice/quinoa (52F - Cold Holding). Operator stated foods are moved to top of reach in when establishment opens at 10am. Operator reduced the amount of product below fill line.
- Wet mop not stored in a manner to allow the mop to dry. Operator hung mop.

- Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed sanitizer at three comp sink testing at 150ppm. Operator drew new sanitizer and added more chemical to bring up to 200ppm.
- Toxic substance/chemical improperly stored. Observed hand lotion stored on prep table and spray bottle of sanitizer hanging on shelf containing coffee. Operator moved both for proper storage.
- Operating with an expired Division of Hotels and Restaurants license. Observed license expired 12/1/2025 and location is open and operating.
- Ready-to-eat, time/temperature control for safety food opened/prepared, frozen and then thawed and held at refrigeration temperature not properly date marked. Observed cooked egg and cooked sausage in reach-in cooler pulled from freezer not properly date marked. Produced is dated 8/16 and 8/17 with today being only 8/13. Educated operator on proper date marking.

Observed cut tomatoes (48F - Cold Holding); cut lettuce (50F - Cold Holding) stored stacked above the fill line in the top section of the reach in cooler at the front counter. The operator emptied half of the contents and stored in the lower portion of the reach in cooler.
- Silverware/utensils stored upright with the food-contact surface up. Observed self-service plastic knives and forks stored with the food contact surface up in the dining area. The operator properly stored the utensils.

Observed an employee wearing gloves handle raw pork then handle a clean dry wiping cloth which was placed on a cutting surface without changing gloves. The employee placed the wiping cloth in laundry. The employee was educated on when to change gloves.
- Time/temperature control for safety food thawed in an improper manner. Observed raw pork thawing at room temperature in the kitchen. The operator turned on cold running water over the pork.
