Upon inspection, observed shell eggs 60f (temperature measured by placing thermometer probe against shell eggs) on grill area table. Eggs have been on table approximately 30 minutes. Assistant manager placed eggs into reach-in cooler during inspection.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Upon inspection, observed precooked chicken 47f in upper make bar cooler # 3 (observed lid open upon arrival, manager stated employee had just made a salad and he was restocking. Recheck of temperature approximately 10 minutes later of 44f). Also observed house-made red pepper ranch 44f, less than 4 hours. Observed radish sprouts 55f in walk-in cooler # 2, less than 4 hours.

- Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the door gaskets for the pizza station reach in cooler soiled with a black slime-like substance.

- Time/temperature control for safety, commercially processed food reheating for hot holding not heated to a minimum temperature of 135 degrees Fahrenheit in all parts. Observed chili at 11:35 (93F - Reheating) in the bain marie in the kitchen. A time mark shows a prep time exceeding 2 hours prior to temperature operator increased the heat for the holding unit. Observed chili at 11:55 (154 - Hot Holding).
- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed butter (55F - Cold Holding) in the top portion of the cold holding unit behind the front counter. The operator stated the butter had been held for approximately 1.5 hours. The operator stored the butter in the lower portion of the cooler.
3941 TAMIAMI TRL #3131, PUNTA GORDA · Charlotte County
- Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw chicken over raw beef. Operator switched the food items.
- Handwash sink not accessible for employee use at all times. Observed shelves blocking the ware washing area handwash sink.
- Equipment and utensils not properly air-dried - wet nesting. Observed multiple containers stacked with no airflow at dish shelf. Operator removed the wet containers from being stacked to allow airflow.
- No handwashing sign provided at a hand sink used by food employees. Observed no handwash sign at ware washing area.
312 W MARION AVE, PUNTA GORDA · Charlotte County
- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed shrimp 47F above the threshold on top of Make Table. Operator placed the food item underneath interior of Make Table to decrease the temperature to 41F or below.
- Carbon dioxide/helium tanks not adequately secured. Observed tank not adequately secured at soda box station. Operator placed a chain around the tank to secure it.
- Open dumpster lid. Observed open lid at dumpster. Operator closed the lid.
- Sanitizing solution for wiping cloths not free of food debris and visible soil. Observed food debris in the sanitizing solution at shelf at cookline. Operator replaced the sanitizer with a new container of sanitizer solution.
- In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed scoop handle touching the flour at container. Operator removed the handle from touching the flour.

Observed shredded cheese held under time as public health control with no time marking near the soup well on the cooks expo line. The operator placed a time mark on the cheese container.
- Handwash sink used for purposes other than handwashing. Observed ice discarded in the handwashing sink at the front counter.
- Portable fire extinguisher gauge in red zone. For reporting purposes only. Observed the fire extinguisher near the office in the red zone for recharging.
- Exit door locked. For reporting purposes only. Observed front entrance and front exit double doors locked on one side for each door.

- Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking. Observed cheese, pepperoni, and ham calzone on time control without a time mark. Operator stated foods were removed from oven 40 minutes prior to inspection.
- Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. t the steam table observed tomato sauce (118F - Hot Holding); cooked meatballs (112F - Hot Holding). Operator stated foods are reheated on burner then moved to steam table. Foods have been on steam table for about an hour. Operator turned up heat.
- Reach-in cooler interior/shelves have accumulation of soil residues and mold like substance.

- Stop Sale issued due to food not being in a wholesome, sound condition. In the stand alone cooler at fire place, Observed 7 portions of fully thawed raw salmon in Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. See stop sale.
- Establishment advertised a specific fish on the menu but served another type of fish. On the menu, dish "traditional ceviche" specifies fresh corvina. Operator could not provide any invoices of corvina fish. Operator stated they are serving swai, operator provided invoices with swai listed. Operator stated they will be reprinting menus.
- Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. On the menu under Guacho starters observed "traditional ceviche" dish, operator stated raw corvina is marinated in lime juice without any heat applied. Operator began to mark menu items to indicate their consumer advisory.
- No paper towels or mechanical hand drying device provided at handwash sink located next to fire grill. Operator provided paper towels.
- Floor soiled/has accumulation of debris in the middle main kitchen area near both reach in coolers.

No proof of parasite destruction for snapper that is served raw under a consumer advisory. Operator stated raw fish is being cured and frozen in house for 10 days. Discussed special processes with operator.
