- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed flat eggs (50F - Cold Holding); liquid eggs (50F - Cold Holding) at RIC across from burners. As per manager, 2 hours. Products were moved to cooling unit for rapid cooling.
- Handwash sink not accessible for employee use at all times. Observed HWS at kitchen blocked by cart. Cart was removed.
- Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed WIF gaskets soiled and prep sink spray hose soiled.
- No conspicuously located ambient air temperature thermometer in holding unit. Observed at RIC across from burners at cook line and RIC at front counter.
- Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Observed RIC across from burners at cookline with an internal temperature of 57F. Any TCS products were removed from unit for rapid cooling.

Observed 15+ live flies at mop sink, flying and landing on dry storage rack where canned goods and spices are stored. Observed 3 live flies on wall next to reach in cooler at back door.

Observed black beans (48F - Cold Holding); diced tomatoes (45F - Cold Holding); corn (45F - Cold Holding); cooked sweet potatoes (45F - Cold Holding); hard boiled eggs (50F - Cold Holding); roasted broccoli (44F - Cold Holding); roasted cauliflower (45F - Cold Holding); shredded cheese (50F - Cold Holding); mozzarella cheese (45F - Cold Holding); blue cheese (45F - Cold Holding) at front counter reach in cooler facing front door. As per operator for one hour. Operator began placing products inside ice bath for rapid cooling.
