- Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed two vacuum breakers missing from two splitters on the threaded hose Bibb next to mop sink outside back door.
- Dented/rusted cans present. See stop sale. Observed 1:can of cranberry sauce and 1 can of unsweetened coconut milk dented along rim of can and pinched in middle of can of coconut milk. See stop sale.
- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed bleu cheese crumbles, cooked sausage (44F-46F - Cold Holding) on door in fridge across from cook line. Operator stated the door was opened a few times today. Advised to keep items in fridge vs on door. Operator placed both items in walk-in-cooler to chill. Retemped at end of inspection at 42F-43F.
- Food-contact surface soiled with food debris, mold-like substance or slime. Observed blue handled soda guns in bar area and some of their holsters soiled. Also, white deflector plate and ice chute area in both large ice machines (one next to dishwasher and other in exterior storage room) soiled.

Ambient thermometer in the unit reads at 50F. Operator states all cheese placed in the unit at 11:00. Cheeses moved to walk-in cooler. Also, observed raw shrimp (47F - Cold Holding); beef (55F - Cold Holding); raw beef ribeye (51F - Cold Holding); cheese (47F - Cold Holding)in left flip top reach in cooler. Ambient thermometer in unit reads at 50F. Per operator all products placed in the unit at 11:00. All products moved to the walk-in cooler. Also, observed sautéed onions (102F/93F - Cold Holding); sautéed peppers (79F - Cold Holding)being held on the kitchen counter with no heat applied. Per operator, peppers and onions prepared 3 hours prior. Operator moved peppers and onions to refrigeration. Also, raw bacon in small refrigerator by the stove burners at 45F/57F. Operator unsure how long bacon was out of temperature. See stop sale.

- At callback observed approximately 6 live roaches in the following areas: 2 on the back of flip top reach-in cooler 2 on wall and on leg of dry food storage shelf 1 next to containers of fryer oil by walk-in-freezer 1 under triple sink. Operator killed and cleaned up as they were pointed out.
- At callback observed approximately 52 rodent droppings in the following areas: 45 in corner area under front and side counter below POS area and 7 in tray at water heater.
- Outer openings not protected during operation and vermin and/or environmental cross contamination present. Observed back door not protected / gap at bottom of screening during operation and vermin present.
- At callback observed approximately 55 dead roaches in the following areas: 6 under chemical shelf 3 in tray of water under reach-in cooler 1 in water of soup well 3 under flip top reach-in cooler 15 in walk in freezer 14 under triple sink 2 under hand wash sink next to triple sink 5 in cabinets in dining area 3 under shelves in ding area 3 in cabinet at hand wash sink at front service area.
