- Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Chlorine dishmachine testing 0ppm after multiple runs.

- Observed in reach in cooler on cook line steamed beans at 62F, Fresh garlic at 68F, cooked noodles at 59F Cold Holding. Cooler has an ambient temperature of 80F. Establishment placed item in reach in cooler from walk in cooler. Manager removed items and placed inside reach walk in cooler.

Observed Raw chicken (52F - Cold Holding); Raw chicken bites (49F-52F - Cold Holding) in reach in drawer cooler on cook line next to fryer. Cooler has an ambient temperature of 54F. Manager stated the product was just rotated out about an hour ago. Employee placed Raw chicken in walk in cooler.

Will turn unit down, keep door closed as much as possible. Other items in unit 40-41F.

- Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee using person phone and then prep food without washing hands
- Gyro meat sliced from cone cooking on rotisserie did not undergo a secondary cook step to ensure all parts, internal and external, reach a minimum temperature of 155 degrees Fahrenheit for 17 seconds or equivalent time/temperature combination as specified in cooking chart. Observed employee slicing raw beef and raw chicken gyro from cone and placing on hummus food tray to package to give to customer without secondary cooking. Customer did not order item to be served under cooked and no consumer advisory sign posted. Person in charge refuse to secondary cooking chart the raw beef that only reached internal temperature of 124F
- Gyro meat in a broiler that is turned off. Observed both chicken gyro machine and beef gyro machines turn off. Person in charge stated they dont have an on and off switch. Machines is rotated by hand to cook and not continuous spinning due to not having that capabilities.

Male employee at the register handled cards and money and immediately after started working with food and cooking equipment, no hand washed occurred, Inspector instructed to wash hands when changing tasks.
