Observed employee walk orders outside to vehicle and walk back in put on gloves to engage in food preparation without first washing hands after touching soiled door. Educated employee and employee washed hands.
- Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed vacuum breaker on wrong side of split. Operator switch to correct side.

Backflow prevention device missing at hose connection at mop sink. The mop sink does not have a built-in backflow prevention device. Manager added a backflow prevention device to the connection during this inspection.

- Toxic substance/chemical improperly stored. Three bottles containing oven cleaner stored on shelf directly beside containers of cooking oil below handwash sink at cookline. Manager removed the chemicals from the shelf during this inspection.
- Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Opened gallon of milk missing date mark inside reach-in cooler in kitchen. Manager stated it was opened on 8/5 and added the proper date mark during this inspection.
- Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. No consumer advisory posted inside establishment or on menu for hamburgers and steaks offered undercooked at establishment. The owner stated the hamburgers and steaks are offered undercooked to customers. Inspector provided a consumer advisory and the manager printed and posted it during this inspection.

- Operating with an expired Division of Hotels and Restaurants license. Establishment is open and serving food to the public with an expired license. License expired 06/01/2026.
- Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Pan containing raw chicken stored directly on top of pan containing raw beef inside reach-in cooler. Employee moved the raw chicken to the bottom shelf during this inspection.
- Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Inside top of refrigerated make-table: shredded lettuce (72F - Ambient Cooling 2 Hours). The cooler has been turned off and has an ambient temperature of 72F. The employee stated she turns the cooler off to keep it from freezing up. She stated the lettuce was cut two hours prior and moved it to another cooler during this inspection. No other TCS foods stored inside the cooler.

All items stocked in morning but lid was removed for lunch service. Manager had employee put flip top lid back on and start putting bags of ice on products.

At wait station drink station, milk (49F - Cold Holding). Operator stated milk has been taken in and out of reach-in cooler.
