9342 OAKHURST RD, SEMINOLE · Pinellas County
- Staff observed washing hands in prep sink in kitchen, discussed using proper sinks for hand washing, employee proceeded to rewash hands in appropriate sink. Same employee was later observed picking up a spatula from the ground and attempted to rinse it off in the hand sink and dunk in a sanitizer bucket. Employee was instructed to properly wash the utensil, but he failed to immediately wash his hands after as well.
- Food-contact surface soiled with food debris, mold-like substance or slime. Deep grooves on front line cutting board closest to order window.

- No handwashing sign provided at a hand sink used by food employees. No handwashing sign over top of hand sink closest to hot line.
- Buildup of food debris/soil residue on equipment door handles. Handles of reach in freezer soiled with grime.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. cooked pasta (45F - Cold Holding); cooked pasta (46F - Cold Holding); cut Tomatoes (46F - Cold Holding); shredded Mozzarella (45F - Cold Holding); cooked meatballs (46F - Cold Holding). Foods were placed in unit from walk in cooler less than 4 hours ago. Foods in walk in coolers are below 40F
- Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Cell phones x2 and ear buds on clean prep table. Operator removed personal belongings from prep tables.
- Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Prep cooler behind front counter nearest to kitchen entrance.

- Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine in bar measured at 0ppm. Instructed operator to use three compartment sink until fixed.
- Establishment unable to be licensed at time of licensing inspection from the Division of Hotels and Restaurants. Operator must obtain a public food service license from the Division of Hotels and Restaurants within 60 days. Submit a license application and payment to DBPR/Bureau of Central Intake Unit, 2601 Blair Stone Rd., Tallahassee, FL 32399-0783. - Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee placed on hat, did not change gloves no hand wash.
