Pizza held in hot holding box on temperature per operator. Pizza placed in box 1.5 hours prior. Operator turned up thermostat.
- Employee switched from working with raw food to ready-to-eat food without washing hands. Employee preparing pizza with raw sausage then handled green peppers without washing hands and changing gloves.
