Also, portable white cutting boards on clean storage shelf next to dishwashing machine and cutting boards on cook line soiled. Also, soda nozzle at bag in box storage shelf in storage room soiled from touching floor. Operator cleaned and properly placed soda nozzle on shelf.
- Equipment or utensils not designed or constructed in a durable manner. Observed door on reach-in cooler across from stove in disrepair/ taped up. Also, gasket on walk-in-cooler torn. Also, multiple handles of spatulas in clean storage pan across from warewashing area burned/ peeling/ chipped/ in disrepair.
- Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket on walk-in-cooler soiled. Also, frame of glass door reach-in cooler in kitchen area has accumulation of debris.
- Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Observed unsealed concrete floor in prep area, cook line, kitchen and front service line.

- Establishment advertised a specific fish on the menu/menu board but served another type of fish. Observed establishment offering Red Snapper on menu displayed on TV above raw fish items. Person in charge provided box snapper originally came in which states Lutjanus malabaricus, commonly known as Malabar Snapper via Seafood List. Person in charge began changing digital menu.
- Clam/mussel/oyster tags not maintained in chronological order according to the last date they were served in the establishment. Observed oyster tags not stored in chronological order in tag box near first kitchen entrance.
- No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towels at hand wash sink between cook line and three compartment sink.
- Employee with no hair restraint while engaging in food preparation. Observed 1 employee engaging in food preparation with no hair restraint.

Manager stated butter placed in cooler thirty minutes prior to temperature being taken. Manager moved butter inside of cooler to reduce temperature to 41F.
- Stop Sale issued due to food not being in a wholesome, sound condition. Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Four packages of salmon stored in reach-in cooler across from cook line.
