- Equipment in poor repair. Observed lid to reach in freezer broken/ not attached to unit.
- Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. Observed 2 frozen packages of salmon placed in water in reach in cooler to begin thawing. Operator removed items from packaging.
