- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In bottom drawer of cook line, pan of raw chicken (41F-47F - Cold Holding). Chicken breast in front at 46F-47F but back of pan at 41-42F. Manager thinks drawer was left open a little bit. Chicken in cooler for approximately 2.5 hours. In stand up cooler in back hallway, multiple bags of raw chicken (45F-46F - Cold Holding). Manager stated chicken was pulled from freezer at 8:30 am and put in cooler. Temperature checked w hours prior was good.
- Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In stand up reach in freezer on cook line, open bag of raw chicken bites on top shelf above egg rolls and French fries. Manager moved chicken bites to bottom of cooler.
- Dead roaches on premises. In back storage room on left side in corner, four dead roaches found on floor. Manager had employees begin clean up and discard.

- Raw animal food stored over/not properly separated from ready-to-eat food. In walk in cooler tub of raw shrimp stored on bucket of sweet and sour sauce. Person in charge moved to appropriate location. In walk in freezer raw chicken stored over raw ground pork on speed rack, no longer commercially packaged.
- Dented/rusted cans present. See stop sale. Observed three cans of soy sauce and one can of oyster sauce with dent along seams.
