- Interior of oven/microwave has accumulation of black substance/grease/food debris. Top plate of both microwaves on the prep line with food debris build up.
- Ice buildup in reach-in freezer and/or walk-in freezer. Two door reach in freezer has ice buildup at top compartment on condenser fan guard.

- Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. In the standup reach in freezer at end of cook line, raw beef burger and Philly steak meat in open containers over bagged ready to eat frozen fries.
- Wiping cloth solution exceeding the maximum concentration allowed stored in a location that could result in the cross contamination of food, equipment, utensils, linens, single-service, or single-use articles. Sanitizer solution by three compartment sink too strong, bleached strip black to white after testing. Operator reset to 100ppm.
- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In flip top reach in cooler opposite the fryers, cut tomatoes (48F - Cold Holding); scallops (51F - Cold Holding); catfish (53F - Cold Holding); fish (49/50F - Cold Holding); shrimp (52F - Cold Holding) employee was placing ice under containers. Inside reach in cooler Mac and cheese in hot container cooling down. Had employee add ice directly to product, at conclusion of inspection; cut tomatoes (40F - Cold Holding); scallops (39F - Cold Holding); catfish (41F - Cold Holding); fish (41/39F - Cold Holding); shrimp (42F - Cold Holding).

Operator stated food has not been in the reach in cooler but a little over an hour, used ice bath and put food into different reach in cooler.
- No written procedures available for use of time as a public health control to hold time/temperature control for safety food. Observed no written plan for the eggs, printed a copy for the operator, went over the form with operator.
- No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.

- Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed a quaternary sanitizer bucket solution exceeded 500 on the cook line. The operator diluted the solution to 200ppm. Observed a chlorine spray bottle solution exceeded 200ppm. The operator diluted the solution to 100ppm.
- Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Observed an employee put fresh basil on a cooked pizza using bare hand contact. No visible cross-contamination observed. Employee washed hands prior to bare hand contact. Educated the employee when bare hand contact is permitted.
- Toxic substance/chemical improperly stored. Observed a container of sanitizer tablets stored on a shelf above a food preparation table on the cook line. The operator properly stored the sanitizer tablets.
