All the following items were stored on ice in the sushi line area Tuna (54F - Cold Holding); Salmon (52F - Cold Holding); Fish roe (53F - Cold Holding); Escola (53F - Cold Holding. Operator stated the items were stored on ice for approximately 1 hour. Operator placed the items in the reach in cooler.
- Non-food grade paper/paper towel used as liner for food container. Cooked lobster and crab stored in pans lined with non food grade paper towel in the reach in cooler on the cook line. Employee removed the paper towels.
- Toxic substance/chemical improperly stored. Containers of drain cleaner stored next to clean pans on a shelf under the grill on the cook line. Employee removed the drain cleaner. Container of bleach stored next to a container of sesame seed on the sushi line. Employee removed the bleach.

Observed ingredients unlidded at pizza make, flying insect observed in pan of minced garlic. See stop sale. Observed approximately 10 flying insects in back kitchen area near dishpit and mop sink area. Person in charge began killing.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Items stored in reach-in cooler next to cook line grill: pork (46F - Cold Holding). Manager placed pork in reach-in freezer to reduce temperature to 41F.
- Raw animal food stored over/not properly separated from ready-to-eat food. Shell eggs stored over sour cream in reach-in cooler next to cook line grill. Manager placed eggs on bottom shelf.
- Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Salsa cooled 0F in ten minutes. Manager placed bag of ice in salsa to reduce temperature to 41F.

Operator had employees place items into other cooler.

Observed grease build up underneath flat top grills, on the side of fryers, and on the side of reach in freezer unit. Observed dish racks in ware washing are heavily soiled.

- Observed establishment remodel inside the kitchen. The kitchen hand washing sink is no longer next to ice machine, it was installed between the kitchen entrance door and the dishwasher. Observed establishment added an extra storage space next door in unit 174. There are 2 reach in freezers where chicken, fish and pork are being stored for the restaurant. There are cases of water, Arizona cans, and soda cans; racks to storage seasonings such as sugar, salt, oregano, sour orange mix and mojo.
