- Raw animal food stored over/not properly separated from ready-to-eat food. In reach in cooler on cook line closest to dish area, breaded raw shrimp on shelf above bagged French fries. Employee moved raw shrimp to bottom shelf.
- Toxic substance/chemical improperly stored. On shelf under metal tortilla holder, two bottles of cleaner stored next to metal chip baskets. Manager moved cleaning liquids.
- Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In drawers on cook line reach in cooler, raw chicken and raw beef stored in top drawer above raw fish with no separation. Manager had employee rearrange.
- Dented/rusted cans present. See stop sale. On can rack in dry storage room, one (1) can of tomato purée dented along side of can. Also, one (1) can of corn dented along side of can.
- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. On bottom shelf under steam table, refried beans (67F - Cold Holding). Employee stated he pulled out of walk-in cooler 10 minutes prior and put on shelf for a second before he starts hitting it up on stove. Also, in top of flip top reach in cooler on cook line, raw shrimp (46F - Cold Holding); shredded cheese (45F - Cold Holding); cut lettuce (46F - Cold Holding); pico (47F - Cold Holding). Manager stated all items put into cooler 2 hours prior but lid has been open for lunch service. Also, in reach in cooler on expo, milk (50F - Cold Holding); sour cream (52F - Cold Holding). Manager stated door has been open during lunch rush.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cheese cake 52F in front counter cooler. Per staff on duty, item in cooler for undetermined amount of time. See stop sale.
- Employee touching ready-to-eat food with their bare hands - food was not being heated as a sole ingredient to 145 degrees F or immediately added to other ingredients to be cooked/heated to the minimum required temperature to allow bare hand contact. Establishment has no approved Alternative Operating Procedure. Cook handling fried fish with hands. Discussed with manager, cook put on gloves during inspection.

- Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Employee water bottle on top of microwave in kitchen area. Manager moved bottle.
2135 SADLER RD, FERNANDINA BEACH · Nassau County
- Establishment using a sanitizer solution that does not meet the requirements specified in 40 CFR 180. Only use single-service items to serve food to customers until approved sanitizer is available for warewashing. Owner states he uses quaternary tablets for three compartment sink but uses unapproved cleaning spray with bleach for food contact surfaces that when tested registered no chlorine. Owner stated he will use quaternary tablets for everything.
- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In refrigerator, hotdogs (45F-46F - Cold Holding). Owner states door has been open and shut for prep this morning. Two packs of hotdogs stored on door of refrigerator.
- Raw animal food stored over/not properly separated from ready-to-eat food. In refrigerator, raw beef stored on container of cooked smoked chicken wings. Owner moved beef to bottom.
- Manager or person in charge lacking proof of food manager certification. Owner and only employee has food handler training through New Habit but does not have copy of manager training.

Observed rice on cook line (66F - Hot Holding) cooked approximately one hour prior to inspection start per person in charge. Discussed and provided time control paper work with person in charge. Person in charge placed rice in walk-in cooler.

- Floor soiled/has accumulation of debris. Observed chicken juice dripping from chicken boxes onto floor. Person in charge stated boxes delivered crushed from recent order. Person in charge had employee start cleaning during inspection.
- Floor area(s) covered with standing water. Observed standing water on floor below cooler on cook line partially cooked person in charge stated it was from thawed shrimp. Person in charge had employee clean up during inspection.
