- Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Dish machine chlorine 0ppm. Inspector instructed manager to discontinue the use of the dish machine for sanitization and utilize the three-compartment sink to sanitize dishes until the dish machine is repaired. Inspector also instructed manager to use the three-compartment sink to sanitize all dishes that have been sanitized using the dishwasher machine today.
- Evidence of mop/cleaning wastewater dumped onto ground. Grey water dumped on ground outside beside mop sink.
- Container of medicine improperly stored. Several bottles of medicine stored on shelf inside bar area above utensils and salt/pepper shakers. Manager removed the medicine from the shelf during this inspection.

- Accumulation of black/green mold-like substance in the interior of the ice machine/bin. Observed black mold like substance inside ice bin.

- The detergent on the dishwasher is not operational. Operator will scrub all utensils and dish ware with soap prior to running through the dishwasher.
- Plumbing system in disrepair. The cold water faucet at the hand washing sink in the kitchen is inoperable.
- There are 3 chest freezers in operation unprotected outside the back door of the establishment.
- There is a build up of ice inside the chest freezers outside the back door and on the walk in freezer door.

- Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. In the under grill drawer cooler raw ground beef is stored above raw whole muscle beef. Operator moved items to proper storage.
- Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. There is no consumer advisory on the bar menu. Provided operator with a copy of the consumer advisory.
- Spray bottle containing toxic substance not labeled. There is an unlabeled chemical spray bottle on the chemical shelf. Operator labeled the chemical spray bottle.
- Handwash sink used for purposes other than handwashing. The hand washing sink by the ice machine had ice dumped inside it. Operator melted the ice.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed a container of chicken noodle soup at 48F and a pan of meat sauce at 49F, operator stated the food has only been in the reach in cooler less than an hour and is going to be reheated for lunch, operator advised a work order has been put in for the reach in cooler.
- No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.
- Handwash sink not accessible for employee use at all times. Observed hand washing sink in the back is blocked by mop bucket and carts, operator moved.
- Employee eating in a food preparation or other restricted area. Observed employee drink on reach in freezer near cook line, operator moved.
- Time/temperature control for safety food thawed in an improper manner. Observed raw bacon thawed in a container of water in sink, operator put under running water.
