- Food-contact surface soiled with food debris, mold-like substance or slime. Observed : stained cutting boards on the cook line. ALSO, the sushi seaweed tin at the bar had an accumulation of shredded seaweed. The container was moved to the kitchen for cleaning.
- Dead roaches on premises. Observed : 4 dead roaches, behind a booth at the far end of the lanai. These were swept up.
- Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed : multiple drinks stored over food throughout the kitchen. These were moved to the bottom shelf or discarded.

- Buildup of food debris/soil residue on equipment door handles. Black and pink substance observed on lid handle and rim along the handle on both doors of the ice machine. Employee cleaned both lids during inspection.
- Standing water in bottom of reach-in-cooler. Standing water observed in bottom of small reach-in cooler at end of the make line and in expo line reach-in cooler.
- Ice buildup in reach-in freezer and/or walk-in freezer. Ice buildup up observed in the walk-in freezer.

Open gallon of milk in walk-in cooler not date marked. Operator states milk opened on Sunday(today is Tuesday) and labeled the milk with the opened date.

Operator stated they recently stocked and had the lid opened and closed during lunch. Operator placed pans of ice in cooler to chill foods. Also, raw scallops, raw shrimp, raw beef (45F-46F - Cold Holding) in reach-in cooler drawers below grill area at Taverna kitchen. Operator stated drawers were opened and closed a bit during lunch. Operator stated he will keep drawers closed to chill foods.
- Non-food grade paper/paper towel used as liner for food container. Observed non food grade storage paper towels used in direct contact in pans with herbs and onions in oyster bar area at Oceana.
