- No paper towels or mechanical hand drying device provided at handwash sink. Observed the handwashing sink at the end of the cook line without paper towels. Operator stocked towels.
- Food-contact surface soiled with food debris, mold-like substance or slime. Observed the motor head on the floor mixer with food debris, grease and rust. Observed the slicer with food debris under the blade guard.
- Floor tiles missing and/or in disrepair. Observed broken and missing floor tiles in front of the walk in cooler.

- Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chlorine concentration at 0 ppm at bar. Operator primed sanitizer and called technician. Note: operator will use working dishmachine in kitchen.
- Toxic substance/chemical improperly stored on top of dishmachine. Operator removed chemical.
- Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb underneath spray table in dishwash area. Operator attached vacuum breaker to hose bibb.

- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Cook line drop in cooler with ice shredded cheddar cheese (50F - Cold Holding); sour cream (54F - Cold Holding); cut tomatoes (50F - Cold Holding) for 1 hour per operator. Operator moved foods deeper into ice and labeled for 4 hour discard.
- Handwash sink not accessible for employee use at all times. Only handwash sink in bar is blocked for full access in far right corner. Only labeled handwash sink in bar area inaccessible near triple sink and ice bin is mostly blocked with large beer reach-in cooler and is no longer readily accessible, and can not be made accessible at time of inspection.

Spoke with employee and manager regarding proper handwash techniques and glove usage.
