- Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Shredded cheese, cut tomatoes and cut lettuce held at 49F in flip top cooler. Bottom door was left open. Operator closed all doors. Rechecked after 30 minutes, No temp change. Operator removed all items to walk in cooler.

- Establishment operating with no potable running water. During a call back inspection, inspector observed establishment open and operating to public with no hot or cold running water throughout entire establishment due to an unpaid water bill.
- Vacuum breaker missing at splitter added to mop sink faucet. Missing vacuum breaker for hoses attached to splitter on mop sink.
