Employee used broom and dust pan and then began handling clean utensils on cook line. Discussed with manager, manager had employee wash hands.
- Nonfood-grade bags used in direct contact with food. Paper bags used to absorb excess oil lining bottom of tub of chicken.
- Raw animal food stored over/not properly separated from ready-to-eat food. Raw chicken over cooked chicken in cook line upright cooler. Manager moved raw chicken to bottom during inspection.
- Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Fried plantains 109F on buffet. Per manager plantains out approximately 30 mins. Manager had staff begin reheating item during inspection.
- Spray bottle containing toxic substance not labeled. Unlabeled pump sprayer in dish area. Per manager pump sprayer contains degreaser.

- Warewashing sanitizing solution in bar three compartment sink exceeding the maximum concentration allowed. Tested at 1130/3010 DDBSA/Lactic acid. Manager diluted solution to 700/1875 during inspection.
- Soap dispenser at handwash sink in meat walk-in cooler not working/unable to dispense soap. Manager replaced with working dispenser during inspection.
- In-use knife/knives stored in cracks between pieces of equipment. Knives stored between prep tables in back line prep area. Manager removed knives at time of inspection.

Per operators, salmon is thawed, then portioned prior to vacuum sealing in regular vacuum packaging bags. See stop sale. Per owner, going forward, salmon would not be reduced oxygen packaged. This violation must be addressed by October 26th.

- Wash solution in spray type hot water sanitizing dishmachine not reaching proper temperature according to manufacturers specifications. Temperature on rinse cycle surface thermometer and heat strip reading 120F. Manager set up triple sink with sanitizer to use as sanitizer.

- Employee switched from working with raw food to ready-to-eat food without washing hands. Stopped employee and had them wash hands. Employee maintained corrected behavior during inspection.
- Ceiling shows damage and is in disrepair. Leaks present in ceiling above kitchen, between fryer and food prep area. Leak is constant and poses a risk for contamination. Area of leak is centrally located in food prep area and traffic through of food and employees high and unavoidable.
